To participate in this free, approved quality improvement project, Ohio nursing homes are asked to sign a memorandum of agreement (MOA) by August 15, 2014, and complete the project requirements.

The project requirements include:

  • Designate a unit (16-40 residents) or units to participate
  • Set own goals
  • Collect data monthly (October 2014 – March 2015), using the Advancing Excellence data tracking tools
    • Consistent Assignment Data Tracking Too<
    • Staff Stability Data Tracking Tool
    • Select QM to correlate and work on improvement-(i.e. Reduce Antipsychotics, decrease falls, decrease in house pressure ulcers).

Project Deadlines

  • Join project by August 15, 2014
  • Attend kickoff September 1, 2014
  • Complete baseline survey by October 1, 2014
  • Collect and report monthly AE data: October 2014-March 2015
  • Complete end-of-project survey: April 15, 2015
