Quality & process improvement tools

The first step to testing change strategies usually involves forming a plan and assembling a team.  Use the tools in this section to develop and test change ideas and then spread “what works” to other appropriate areas, shifts, departments, and/or staff members.

Learn about making sustainable changes using rapid-cycle testing (also known as the Plan-Do-Study-Act or PDSA cycle).  Use the worksheet on the last two pages to document notes throughout the rapid-cycle test.

View this presentation to learn more (or teach more) about using rapid-cycle testing to implement change strategies.

PDF (2.0 MB)        Microsoft PowerPoint (407 KB)

A guide for running healthcare meetings by assigning team members to various meeting roles.  An agenda template for this format is also included.                       PDF (39 KB)                         Microsoft Word (174 KB)

A template for writing a plan of action to either create change to improve quality or sustain/reinforce successful strategies.

PDF (38 KB)                         Microsoft Word (125 KB)

Before making changes to a current process, it is important to first map out how things are currently done, including all forms and staff members involved.  Use this worksheet as a guide to map out current processes.  From Rhode Island Quality Partners, used with permission.

PDF (46 KB)                         Microsoft Word (128 KB)

The root cause analysis allows you to identify the “root” of the problem you discover in your process -- where and why the problem exists.  Use this worksheet and the cause-and-effect (fishbone) diagram to get to the root cause of the problem.  From Rhode Island Quality Partners, used with permission.                   PDF (43 KB)                         Microsoft Word (226 KB)

Identify a manageable change based on the outcome of root-cause analysis.  From Rhode Island Quality Partners, used with permission

PDF (37 KB)                         Microsoft Word (124 KB).

Use this worksheet to explain details surrounding changes in processes and procedures.  From Rhode Island Quality Partners, used with permission.                   PDF (38 KB)                         Microsoft Word (126 KB)

Pilot testing gives your team a chance to see how to implement a change on a small-scale.  Use this worksheet to assign roles for pilot testing new change strategies.  From Rhode Island Quality Partners, used with permission.                   PDF (28 KB)                         Microsoft Word (122 KB)

Evaluating the pilot test allows your team to organize observations that the team has made through the pilot test.  Evaluation also includes collecting data to check whether the change has helped you reach your goal.  Use this worksheet to evaluate your pilot-test.  From Rhode Island Quality Partners, used with permission.  PDF (40 KB)       Microsoft Word (132 KB)

Monitoring the implemented change allows your team to evaluate, on an ongoing basis, whether the implemented change has made an impact on overall care delivery.  Use this worksheet as a guide for monitoring and evaluating progress.  From Rhode Island Quality Partners, used with permission.                   PDF (46 KB)                         Microsoft Word (131 KB)
