Action Pact, Inc., is a coalition of trainers, consultants, and educators who focus their efforts towards helping people work better for themselves and their organizations. Action Pact does this by working with its customers to redesign the way the organization itself functions, restructuring more traditional top-down management into a collaborative effort of self-governing teams. This allows people to shed the confining role of a traditional employee, giving them opportunities to be leaders in their own right.  Link to the Web site.

Formed in 2005 by stakeholders in the Ohio long-term care industry, the Ohio Person-Centered Care Coalition influences and supports transformational culture change in long-term care environments. The coalition works to create a long-term care environment where all individuals can experience meaning and purpose.  Link to the Web site.

The nonprofit Paraprofessional Healthcare Institute (PHI) focuses on strengthening the direct-care workforce within our nation’s long-term care system through developing innovative approaches to recruitment, training, and supervision; consumer-centered caregiving practices; and effective public policy.  Link to the Web site.

The PEAK (Promoting Excellent Alternatives in Kansas) nursing home initiative was created by the Kansas Department on Aging in order to promote more social, non-traditional models of long-term care.

Based upon a common desire for change, the SAGE organization has evolved from a few volunteers into a multi-state organization with hundreds of members. SAGE currently has an active National Federation steering committee and local chapters in six states: WI, OH, UT, KS, IN, and MI.
