Learn more about the concept of person-centered care and different culture change philosophies of nursing home industry leaders. Read compelling support and rationale for culture change from the industry’s top leaders, including long-term care regulatory officials at the Centers for Medicare & Medicaid Services. Download a helpful change-readiness assessment, progress self-assessments, or surveys for staff, residents, and families. These tools will help you determine your progress toward providing care that is truly person-centered – and your readiness to implement new changes. Access an archive of change strategies for person-centered care in areas such as bathing, dining, death and dying, and sleeping/waking. Download ideas for recruiting and orienting employees, as well as how to provide employees with consistent assignments. Education & communication tools Use the communication tools to facilitate discussion among staff, residents, and families.
